Introduction
There is no Robot in our country so we have to depend on Laparoscopy for MIS. The first objective of radical prostatectomy (RP) is cancer control. However, the success of RP should also be measured by the most unforgiving long-term morbidity outcomes and by the maintenance of quality-of-life. Incontinence and erectile dysfunction have clearly negative repercussions on a patient’s quality-of-life although most men are usually satisfied with treatment and accept these subsequent disorders
Materials and Methods
Between January,2021 to December, 2024. Twenty one patients underwent Laparoscopic Radical Prostatectomy. Those who were impotent or incontinent before the surgery were excluded. All patients prospectively completed objective, self-administered questionnaires before the medical visit, concerning their voiding and sexual disorders, before and after surgery. Biochemical recurrence was defined as any detectable serum PSA (≥0.2 ng/mL). Potency was defined as the ability to achieve an erection sufficient for penetration with or without the use of phosphodiesterase-5 enzyme inhibitor. Urinary continence was defined as absence, or occasional use, of a pad for anticipated vigorous activity.
Results
Median age and preoperative PSA level were 64.7years and 18.6 ng/mL, respectively. A trifecta outcome was achieved in six patients, which is 29% of total patient. Three patients were margin positive (14%) and eighteen patients (86%) were cancer free. Urinary incontinence persist in four (19%) patients after 6 months of operation.
Conclusions
Four years after the procedure, trifecta outcome is achieved in six patients who remained potent and continent. Combined results of good cancer control and continence recovery are reported in 80% of patients, regardless of erectile function. Seventeen (81%) patient suffered from ED after 6 months of operation.
