Introduction
Conventional penile venous surgery and enhancement have been controversial for centuries. Recently, a de novo penile fibrovascular assembly has been extensively studied, leading to a novel strategy that has been applied for decades. This approach is based on a revolutionary understanding of penile anatomy, combining penile venous stripping surgery (PVS) with factual penile enhancement (FPE). We conducted a retrospective study to evaluate the outcomes of this innovative method.
Materials and Methods
From 2013 to 2025, patients with refractory impotence and veno-occlusive dysfunction (VOD) sought PVS and penile girth enhancement (PPGE). Eighteen men underwent PVS and FPE, with PVS involving vein stripping and FPE using a venous stripe and either autologous venous walls (AVW) or Surgiform (SF). The control group received follow-up without surgery. Penile girth and length were measured pre- and postoperatively, with intracorporeal retention assessed via radio-opaque contrast. IIEF-5 and EHS scores were evaluated annually through online follow-up.
Results

IIEF-5 and EHS scores were evaluated annually via internet-based follow-up.
Conclusions
This study shows that combining penile venous stripping surgery (PVS) with factual penile enhancement (FPE) effectively improves erectile function and penile girth in patients with veno-occlusive dysfunction (VOD). Significant improvements in IIEF-5 and EHS scores, along with enhanced cavernosogram radiopacity, confirm the approach's efficacy. Larger sample sizes and extended follow-up are needed for further validation.
