Introduction
Finding factors that can predict the survival rate of transplanted kidneys has been of interest to researchers since the past. In the present study, we examined the relationship between the findings of donor KDPI (Kidney Donor Profile Index) and zero-time biopsy (ZBT) with the one-year function and survival of the transplanted kidney.
Materials and Methods
In this prospective cohort, which was conducted between 2019 and 2021, all patients who were candidates for cadaveric kidney donation and candidates for transplantation in Montasarih Hospital, Mashhad were evaluated by KDPI. Then, during the organ harvesting operation, a needle biopsy of the kidney was taken in the operating room and sent for pathology. After kidney transplantation, all patients were treated and followed up in a routine and standard way. The serum creatinine level was investigated and recorded in one week, one month, three months and one year after grafting. Finally, the obtained information was analyzed using SPSS software.
Results
A total of 80 kidney transplant recipients were included in the study, of which 41 (51.2%) were male and the rest were female. The average age of the patients included in the study was 31.78 ± 9.23 years. None of the patients in our study had graft rejection during the follow-up period. The median donor KDPI was 0.86 (0.79-0.91). Creatinine in the first week had no significant correlation with KDPI (p=0.362, r=0.103), but creatinine in the first month (p=0.006, r=0.304), creatinine in the third month (p=0.002, r=0.336) and creatinine in the first year (0.001 p<, r=0.452) had a significant and direct correlation with KDPI. Also, creatinine in the first week (p=0.005, r=0.311), creatinine in the first month (p<0.001, r=0.512), creatinine in the third month (p<0.001, r=0.614) and creatinine in the first year (p<0.001, r=0.616) had a significant and direct correlation with pathology grade
Conclusions
Based on the results obtained in the present study, the increase in KDPI and the increase in the pathology grade in zero-time biopsy are associated with the decrease in the function of the transplanted kidney after one year, and these factors can be used to select suitable transplant donors.
