Penile zipper entrapment, while uncommon, can lead to significant discomfort and complications, especially in adults. This case involves a 44-year-old uncircumcised man who experienced a delay in seeking medical help, ultimately requiring surgery due to tissue damage. Understanding the management and comparing it with existing research helps highlight the importance of timely treatment.
(Case Summary) The patient, a 44-year-old man with no chronic illnesses, had his penis foreskin trapped in a zipper. He delayed seeking help for 24 hours, arriving at the urology clinic with parts of his pants cut away, leaving the zipper and fabric stuck. He refused emergency surgery that night for personal reasons, and surgery was performed the next morning. Initial attempts to cut the zipper failed due to necrosis, leading to an elliptical excision. One week later, his wound healed well, with normal erectile and urinary functions.
(Literature Comparison) Research shows non-surgical methods, like cutting the zipper's median bar, are often effective, especially in children, but delayed cases in adults may require surgery, as seen here. A similar case of a 64-year-old circumcised man used a ring cutter successfully, contrasting with our patient's outcome, possibly due to uncircumcised status and delay. Literature also suggests elective circumcision for prevention in high-risk cases, an unexpected consideration for adults.
This case underscores the need for early medical attention to avoid complications. Clinicians should adapt treatment based on individual factors, and further research could explore preventive strategies for adults.
