The outcome of this study was to investigate the success and complication rate of EndoFast Reliant system is a new technology for the treatment of pelvic organ prolapse (POP) repair, with a 5-year follow-up. This system employs a novel technology that enables a minimally invasive technique for reinforcement of the vaginal wall using IBI’s Spider Fastener™ technology. The procedure is performed trans-vaginally with direct palpation and visualization, without blind trocar passage. Therefore less trauma and damage to adjacent organs is expected as compared to current conventional techniques. The system consists of precut mesh and soft-tissue fasteners. The fasteners are easily deployed through narrow spaces into soft tissues, and can support an average weight of 1.0 kg each (far more than required in a typical Pelvic Organ Prolapse repair procedure). The system also includes an extraction device that allows intra-operative reversibility of fixation without causing damage to the tissue or the mesh. The EndoFast Reliant™ system is a sterile, single-use system consisting of the following components: Operating handle and 5 Fixation Devices preloaded with the Spider Fastener. The Spider Fastener is provided with a Fastener Retrieval Unit that enables facile retrieval of the deployed Fastener. An Extraction Device, used with the Fastener Retrieval Unit to ensure facile retrieval of the deployed Fastener. The mesh used with the system is a pre cut Polypropylene monofilament Surgical Mesh.
This study was comprised 24 female patients with pelvic organ prolapse (POP) who underwent vaginal POP repair.All operations were performed between Jully 2013 and June 2014.Total follow-up was 60 mounts(1 month,3 months,6 months,12 months,24 months,36 months,48 months,60 months).All patients were evaluated with a clical history, POP-Q measures,pelvic ultrasound,BMİ , questionnaires on symptoms (PFDI) and quality of life scoring system.
Total follow-up 60 months.The mean age 51 years old(47-73). Mean body mass index was 26,7 (25,3-33,7) with 18 patients BMİ<30 and 6 patients with BMİ>30.There were no statistically significant differences between in two BMİ groups. There were no major intraoperative complications like bladder perforation, vasculer injury and nevre damage. Prolapse has been anatomically resolved in 82 % at 5-year follow-up.Three patients presented stage 1 nonsymptomatic prolapse . Three cases (12,5 %) had de novo stress urinary incontinence (SUI) and two cases had de novo urgency (8,3 %) were diagnosed and treated .There was no mesh extrusion and migration of fasteners or mesh. No patients was urinary retention, postop groin pain and dyspareunia.There were no patients with postoperative sexual dysfunction.
The EndoFast Reliant system was found hihg success rate at 5 year follow-up.The procedure was easy to learn and lower complication rate.
