Ectopic kidney is the failure of the kidney to ascend to its normal anatomic location. It is commonly associated with urinary tract abnormalities such as ureteropelvic junction obstruction and presence of renal stones. Percutaneous nephrolithotomy is the treatment if choice for patients with large or complex renal stones. However, in cases of ectopic pelvic kidneys, it is usually done with the aid of laparoscopy in the supine position due to its abnormal location, rotation and aberrant vasculature. This is the first reported case of the procedure in the country.
(case report): A female in her 30s presented with a two-week intermittent, right lower quadrant pain with no associated signs and symptoms. CT stonogram showed an ectopic right kidney at the level of L3 to L5 with a 2.0 x 1.6 x 1.4 cm middle calyceal calculus (1478 HU). Patient underwent cystoscopy, retrograde pyelography, laparoscopic-assisted percutaneous nephrolithotomy on the right kidney with maximal stone clearance. Patient tolerated the procedure well and was discharged on the fourth post-operative day.

Laparoscopic-assisted percutaneous nephrolithotomy is deemed safe and effective for patients with large or complex renal stone/s in the ectopic kidney with excellent stone-free rates and minimal complications.
