Introduction
Prostate cancer is the most common noncutaneous malignancy with incidence of 19% worldwide. Active surveillance is preferred on very low- and low - risk prostate cancer. National Comprehensive Cancer Network guidelines for prostate cancer recommend active surveillance as a viable option for patients with favorable intermediate-risk prostate cancer. The objective of the study is to conduct a national survey on utilization of Filipino urologist of active surveillance for low- and intermediate-risk prostate cancer.
Materials and Methods
Forms were sent to the 467 Filipino urologist (total number of Filipino urologist based on Philippine Urological Association registry as of 2024) from August 2024 to September 2024. The items included were years of experience as a urologist, sex, workplace, treatment equipment owned, specialty area of daily practice, specialty area of urological cancer, and six hypothetical cases of AS. The cases were categorized by the following Gleason scores: 3 + 3 low-risk prostate cancer, 3 + 4 intermediate-risk, and 4 + 3 intermediate-risk, with or without comorbidities for each case. Comorbidities were defined as cardiovascular diseases or illnesses warranting anticoagulant therapy
Results
Total of 136 Filipino urologists responded (29.1%) response rate. Percentages of proposed AS for low risk(no comorbidity), low risk(with comorbidity), favorable intermediate-risk(no comorbidity), favorable intermediate risk (with comorbidity), unfavorable intermediate risk (no comorbidity), and unfavorable intermediate risk (with comorbidity) were 77%, 79%, 30%, 56%, 21%, and 38%, respectively. Analysis of the correspondents' backgrounds showed that the more the urologists' years of experience, the less they were to advise AS of low-risk patients
Conclusions
Active surveillance is widely accepted by Filipino urologists for patients with low-risk prostate cancer. However, fewer than 50% of urologists supported AS for intermediate-risk prostate cancer patients.
