Testicular Germ Cell Tumors (TGCT) are the most common solid tumors in young men, typically affecting individuals aged 15 to 35. TGCT is a highly treatable malignancy with an excellent prognosis. The management usually involves surgery, chemotherapy, and radiotherapy, depending on the stage and presence of metastasis. The study aims were to evaluate the Clinical presentations, functional and oncological outcomes of patients diagnosed and treated for TGCT at our center. We analyze the treatment modalities, assess post-treatment overall health-related quality of life, and identify prognostic factors that influence outcomes. This study aims not only to enhance our understanding of the disease's impact on Omani men but also to directly inform clinical practices and patient management strategies within the Omani context. It ultimately aims to improve health outcomes and quality of life for affected individuals.
This retrospective study included males > 14 years with testicular germ cell tumors diagnosed and treated at Sultan Qaboos University Hospital from 01 January to 31 July 2023. Patients data were collected from hospital ‘TrackCare’ Software System for the demographics details, including age at presentation, staging, treatment, follow up and outcomes. Review current data on TGCT incidence, risk factors, and demographics. Also discussed the standard treatment protocols including radical orchiectomy, retroperitoneal lymph node dissection (RPLND), chemotherapy, and surveillance. We summarized the treatment outcomes including overall survival rates, recurrence rates, and long-term complications. An Excel sheet and Statistical Package for Social Science (IBM SPSS) software was used to analyze the data.
We analyzed 21 cases of TGCTs among 29 registered patients. The mean age was 28.7 years and mostly were originated from the different regions of Oman (93%). The most common clinical presentation was painless, palpable testicular swelling among 17(81%). Seminomas and non-seminomas germ cell tumors were accounts for 52.4% and 47.6%, respectively. All patients underwent radical inguinal orchidectomy. 61.9% of patients received Adjuvant chemotherapy, with bleomycin, etoposide, and cisplatin (BEP) being the most common agents. Twenty (95.2%) patients were disease-free at their last follow-up.
This study concludes that early diagnosis and prompt treatment through standard radical inguinal orchiectomy led to improved outcomes, and all patients should be encouraged to consider sperm banking. There is a critical need for enhanced health education to ensure that patients seek care sooner and receive the appropriate medical attention. Our current data from a single center can inspire other facilities to evaluate and report their oncological outcomes, ensuring that we deliver the highest standard of care to all patients in the Sultanate of Oman.
