Introduction
Percutaneous nephrolithotomy (PCNL) is the gold standard treatment for renal stone larger than 2 cm. Mini PCNL has been proposed as an alternative to reduce the risk of complications. This study examines the efficacy and safety of mini PCNL versus standard PCNL.
Materials and Methods
This study evaluated the PCNL procedures conducted in Queen Mary Hospital, Tuen Mun Hospital, Tung Wah Hospital and Pok Oi Hospital from 1st January 2017 to 31st March 2023. Patient demographics, number of tracts, stone number and size, surgical outcomes, and complications were analyzed. Patients with stone size of less than <400mm2 were included.
Results
A total of 353 PCNL procedures were evaluated. There were 160 (45.3%) mini PCNL and 193 (54.7%) standard PCNL. Patient demographics and stone count were similar. Mini PCNL group showed a smaller pre-operative stone diameter (18.4 mm vs. 20.7 mm, P<0.001) and size (179.9mm2 vs, 220.9mm2, P<0.001). Similar stone-free rate and number of additional treatments were noted. However, mini PCNL had a longer operation time (128.5 vs. 113.1 minutes, P<0.001). Mini PCNL demonstrated less blood loss in terms of hemoglobin drop (0.92 vs. 1.24 g/dL, P=0.01) but the transfusion rates were comparable. The incidences of sepsis, fever, and renal impairment were similar.
Conclusions
Mini PCNL is a reliable alternative to standard PCNL.
