Currently, the problem with referral system includes several factors,such as issues related to data transmission, which may be incomplete, incorrect, or difficult to access. These problems create inconsistencies in patient treatment and affect communication and coordination between hospitals, leading to gaps in patient care We have developed a consultation and referral system for patients with solid tumors (E-consult) to facilitate health personnel in providing consultation and co-management of cancer patients in various hospitals within the referral network of Songklanagarind Hospital. This initiative aims to ensure that patients receive appropriate cancer treatment without having to visit the hospital unnecessarily.We aim to study the outcomes of E-consult, focusing on the following aspects : waiting time for treatment, the number of hospital visits to Songklanagarind Hospital. The findings will serve as qualitative data for further development, improvement, and management of the program
A retro-prospective study, We randomly assigned cancer patients who received continued treatment at Songklanagarind Hospital through the E-consult system and the standard referral system from October 2015 to December 2018, as well as patients who will continue to use the service in the future. The primary end point was waiting time for treatment compared between two groups.The number of hospital visits to Songklanagarind hospital was secondary end point
A Total of 500 patients were randomly assign in E-consult system group and 500 patients were randomly assign in standard referral system group. Standard referral system was associated with significantly longer waiting time than E-Consult (76 vs 65 days; P = 0.001 [MWU]) The number of hospital visits to Songklanagarind hospital was significantly lower in the E-Consult system than Standard referral (2 vs 5 times; P = 0.001 [MWU])
E-Consult system showed excellent result in reducing waiting time and number of hospital visits for cancer patients compared to Standared referral system. Which could be beneficial for these patients who have time constraints and are going to face increasing burdens in the future
