The aim of our study was to evaluate the possibility and safety of this type of surgery as a preparation for future renal transplantation.
38 symptomatic patients with terminal chronic kidney disease due to Autosomal dominant polycystic kidney disease (ADPKD) underwent simultaneous bilateral laparoscopic nephrectomy from May 2018 to December 2024. 2 video cases are presented.
Mean operative time was 185,5 (120 – 290) minutes.There were no intraoperative complications. Postoperative complications Clavien-Dindo >3 were noted in 3 patients (A-V fistula thrombosis, pneumonia and postoperative hematoma formation due to hypocoagulation syndrome). Mean Hospital stay was – 6.4 days. No signs of adrenal deficiency was noted in any patient. 35 patients underwent kidney transplantation from deceased donors. Posttransplant inflammatory complications were noted in 2 patients, that is significantly less then in comparable group of transplant patients with ADPKD and without pretransplant nephrectomy done.
Simultaneous bilateral laparoscopic nephrectomy of polycystic kidneys in hemodialyzed patients is feasible and safe procedure in this severe group of patients that allows to decrease the rate of posttransplant inflammatory complications.
