Introduction
Prostate cancer is a leading cause of cancer-related deaths in Western countries, mainly affecting men aged 45 to 60. Globally, over 1.4 million new cases have been reported by WHO. Diagnosis involves PSA testing, MRI, and biopsy, with risk factors including age, obesity, race, and family history. While surgery and radiation remain standard treatments, their side effects drive interest in alternatives like Prostatic Artery Embolization (PAE), commonly used for benign prostatic obstruction. However, its role in prostate cancer remains unclear. This systematic review aims to evaluate the efficacy of PAE as a palliative strategy in managing clinical outcomes of prostate cancer.
Materials and Methods
We searched for English-language full-text literature from Pubmed, Cochrane, Wiley Library, Proquest, SpringerLink, and ScienceDirect databases from January 2013 to Desember 2023. Based on the 1442 journals identified in this study, the number of evaluated articles is 6 articles.
Results
Six studies show that PAE, performed via femoral artery embolization under local anesthesia, offers shorter hospital stays and better tolerance than TURP. It significantly reduces IPSS and shows improvement at 1 and 6 months. While histology reveals necrosis, viable cancer cells may remain. PAE may serve as an adjunct to reduce radiation exposure in localized prostate cancer therapy.
Conclusions
PAE is a minimally invasive treatment for LUTS related to BPH and its potential in managing PCa. PAE, utilizing femoral artery occlusion, shows significant short and medium-term reductions in IPSS. While there's notable success in PCa management and reduced radiation doses, further research is essential for a comprehensive understanding of PAE's efficacy in localized PCa treatment.
