Treating BPH men is the bread-and-butter job of most urologists. Even though transurethral resection of prostate adenoma (TURP; with monopolar/bipolar electrocautery or various laser energy) is still the platinum standard in surgical treatment for enlarged prostate with bladder outlet obstruction, they are not without side effects. During the past 10-15 years there have been multiple novel minimally invasive surgical treatment (MIST) options available throughout the world that offer similar and long-lasting outcomes, yet with fewer adverse effects. Guidelines from prudent academic societies like AUA and EAU have adopted these new treatment choices into their updated BPH guidelines. 

For a urologist involved in offering MIST and other new BPH therapy for his patients with bladder outlet obstruction, should he or she learn just one treatment modality and use it for all/majority of his patients, or should he/she learn the secret recipes of multiple treatments and tailor the therapy decision according the disease and anatomical details of his specific patients?

With so many treatment alternatives available, it is no longer the old ‘one size fit all’ style TURP treatment algorithm. Besides the complexity of learning the tips and tricks of various new modalities, letting the interested patients/family understand the RBAPC of each treatment, how to individualize a custom-made tailoring of the decision-making to benefit the BPH men best is becoming more challenging for both the patients and the urologists alike.

In 2 of 90-min multi-side crossfire sessions, experts with great experience in utilizing various treatments will first give a brief introduction of his armamentarium, then several BPH cases with different disease severity and prostate anatomy will be presented, and each panelist will answer if his/her equipment/instrument is a good choice for that case; why; any tips/tricks to do that specific case beautifully? After all panelists’ presentation, the commenter(s)/ moderators will challenge them with questions. Finally, the audience will vote their preference. The 1st session will be focused on BPH with BOO <= 80/100 gm, and UroLift, Resum, iTIND, Thulium or Holmium laser, and aquablation will be discussed. The 2nd session will be focused on symptomatic BPH> 80/100 gm, and Thulium or Holmium laser, aquablation, multiport or single port robotic adenomectomy will be discussed.
